This surgery is performed using the same tools and equipment as neuro-microsurgery.
The skin of the lower eyelid is not incised and no scars are left. A small window of about 3mm is made in the conjunctiva of the lower eyelid, and the surgery reduces the necessary and sufficient amount of intraorbital fat that causes Bulge and Sagging.
The procedure takes 15 minutes on one side and 30 minutes on both sides.
By using neuro-microsurgical techniques, bleeding during surgery can be kept to a minimum, allowing for delicate and safe surgery with minimal invasion of living tissue.

TCB Surgery
A procedure that addresses puffiness and sagging under the eyes by removing excess orbital fat through a small incision made in the conjunctiva (inner lower eyelid).
The fat is carefully reduced from behind the eyelid without cutting the outer skin, helping restore a smoother, more youthful contour.

Just like in standard neurosurgical procedures, a neurosurgical operating microscope is used. This allows for an extremely delicate and precise surgery, as the skin under the eyes is handled without direct contact, minimizing trauma and ensuring gentle, tissue-preserving care.
Treatment Flow
Preoperative markings are made.
Nitrous oxide anesthesia is started. Nitrous oxide has a sedative and analgesic effect, so you can relax during the procedure.
The mucosa is gently pulled with a thin thread the size of a hair, so the procedure can be performed with your eyes closed.
A window of a few millimeters is created in the mucosa with microsurgical scissors.
In this procedure, fat is not removed in large amounts in the form of so-called “defatting,” but the amount that naturally protrudes from the mucosa window is optimally reduced.
The procedure takes about 10 to 15 minutes on each side. There is no internal bleeding immediately after surgery. The window in the mucosa will close naturally in a few days without sutures.

Day After the Procedure
Around five hours after the procedure, the area under my eyes started to turn bluish. When I woke up the next morning, I had a lot of eye discharge. The bluish tint from the day before had faded, but the swelling was still quite noticeable.



Day 2
Some bruising appeared and spread around my left eye. It felt a bit better than the day before, but I still had a slight discomfort when blinking.



Day 3
My eyelids felt slightly heavy, but when I wore glasses, most people didn’t notice anything unusual.



Day 4
The area under my eyes began to take on a slightly yellowish tone. With some concealer, it was easily covered and not noticeable at all.
As the changes after Day 4 were difficult to capture in photos, we’ve skipped ahead to the one-week mark.



One Week Later
One week has passed. Most of the discoloration and bruising has faded. There’s still a bit of puffiness in my cheeks, but that actually made them look fuller—in a good way. Some people even told me, “Not just your eyes, but your whole face looks younger.”
